Birth Certificates
To obtain a copy of a Birth Certificate, a Birth Certificate Application Form (PDF) needs to be completed by the appropriate individual.
Once you have completed the Birth Certificate Application, please bring a photo ID along with the application to the Town Office and a certified Birth Certificate will be issued.
- 1st copy of Birth Certificate - $15
- 2nd copy of Birth Certificate - $6 (If within the same request for 1st copy)
Please bring with you the following information:

Death Certificates in Memoriam
To obtain a copy of a Death Certificate, a Death Certificate Application Form (PDF) needs to be completed by the appropriate individual.
Once you have completed the Death Certificate Application, please bring a photo ID along with the application to the Town Office and a certified Death Certificate will be issued.
- 1st copy of Death Certificate - $15
- 2nd copy of Death Certificate - $6 (If within the same request for 1st copy)

Marriage Licenses
To obtain a marriage license, a Marriage Intentions Form (PDF) needs to be completed. This must be signed by both parties together in front of the Town Clerk.
Once you have completed the Marriage Intentions, the Town Clerk will process the license.
Please bring with you the following information:
- Marriage Intentions Form
- Photo ID for each party
If either party has been divorced, a Certified Divorce Decree with raised or colored seal must be presented.
Note: Your Marriage License will expire after 90 days of the issue date.
To obtain a certified copy of a Marriage License, a Marriage Certificate Application Form (PDF) needs to be completed by the appropriate individual.
- 1st copy of Certified Marriage License - $15
- 2nd copy of Certified Marriage License - $6 (If within the same request for 1st copy)
Please be aware that certified copies may require a 24-hour turnaround
Note: This process can take a ½ hour to 1 hour to complete.
For non-residents of Maine, it is recommended that you obtain your license in the town where the marriage will take place.
- How do I obtain copies of birth, death, marriage or domestic partnership records?

Records may be obtained directly from the Town Office. You will need to provide positive identification, and if requesting the record of a parent or grandparent and you are not a registered genealogist, proof of direct lineage.
- Why do I need to provide positive identification when requesting a record?

While most requests for vital records are honest attempts to obtain one's own personal documentation, some are not; some are attempts to obtain information and documents needed to assume another person's identity. These documents can be used to obtain a driver's license, state photo ID, social security card, and passport under the assumed name, opening the door to credit card, bank and tax fraud; mail theft, and social security and insurance fraud. The requirement that you provide identification when requesting a copy of record helps to protect you and your family from this type of crime.
- What are acceptable forms of identification?

Acceptable forms of identification include a driver's license, passport, or other government-issued photo identification.
- What if I do not have an acceptable photo ID?

If you do not have acceptable photo identification, you may present two items with your name on it from the following list: a utility bill, a bank statement, a car registration, a copy of an income tax return, a personal check with address, a previously issued vital record or marriage license, a letter from a government agency requesting a vital record (for example, the Maine Department of Health and Human Services), a Department of Corrections identification card, a Social Security card, a DD214, a hospital birth worksheet, a license or rental agreement, a pay stub (W-2), a voter registration card, and a school or employee photo ID. Other forms of identification listing your name, date of birth, and address may also be considered.
- How much do certified copies cost?

The first copy is $15, and additional copies of the same vital record are $6 if purchased at the same time.
